Suggest Treatment For Symptoms Of Acute Chronic Cystitis

Question: ( 1a ) Can an undiagnosed URINARY TRACT INFECTION endure for over a year, or perhaps 1 1/2 yrs, without becoming elevated to an intollerable level of pain and discomfort, at which point the patient finally seeks help?
( 1b ) When stringy MUCOUS is being expressed within urine (approximately 50% of all urination episodes), could this symptom sometimes NOT simply be the "lining of the bladder" being shed? Could this symptom of stringy 8" to 10" milky mucous being excreted (within urination) possibly indicate BLADDER CANCER?
( 1c ) After a year of expressing urine that also has a string of murky mucous being excreted (at the same time), and once this white/milky mucous SUDDENLY CHANGES COLOR and suddenly has the presence of DARK BLOOD (old blood) inside the mucous strings that are being excreted (dark brownish red blood streaks inside the now pinkish mucous strings, running down the full length of the mucous strings, and depositing into a small pile at the bottom of the toilet bowl), could this BLOOD indicate progression toward eventual or existing BLADDER CANCER? If so, what tests are necessary to determine BLADDER CANCER, rather than determining a simple case of URINARY TRACT INFECTION? What are realistic time lines for diagnosis and treatment? Please discuss all. Thank you kindly.

Regarding my question pertaining to differenciating between URINARY TRACT INFECTION and possible BLADDER CANCER, I probably should have mentioned that I am also a very long-time DIABETIC. Here in Canada, we are advised that all Diabetics are seriously prone to developing Bladder Cancer. ALSO, I endure very severe LEG INFLAMATION and unbearable PAIN in the lower half of my left leg, due to VARICOSE VEINS AND SPIDER VEINS, but not "ulcerated" veneous condition, at least not as yet. The heat radiating from left leg, and blue/red discoloration is very severe, impeding mobility and sleep. No other illnesses, nor maladies, are present.

Thanks for your query,based on the facts that you have posted it appears that you have Acute on Chronic Cystitis.
I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Is your Diabetes under control?
2) Are you taking any medications especially blood thinners like Aspirin ?
3) Do you have other urinary symptoms like frequency,urgency,burning urination etc.?
Based on the facts that you have posted I would say that you have chronic Cystitis with recent flare up of infection (Acute on Chronic Cystitis).

As regards presence of blood and mucus streaks are suggestive of shedding off of the bladder mucosa(MUCK) .It is unlikely to be due to Bladder Cancer.Bladder cancer always presents with passing of XXXXXXX fresh blood (Gross Hematuria).

Please consult qualified Urologist for clinical assessment and get following basic tests done to confirm the diagnosis and start treatment at earliest .
1) Routine Urine test and Urine culture.
2) Ultrasound Scanning of abdomen and pelvis.
3) C.T scan of abdomen
4) Endoscopic examination of Bladder (Cystoscopy).(This is very important from the point of view of Diagnosis))
5) Urine Cytology.

In the mean while take broad spectrum antibiotics like Cefexime along with urinary antiseptic like Nitrofurantoin twice daily and urine alkaliser thrice daily.Later on switch on to appropriate antibiotics as per culture report.

Get the prescription of medicines from your family Physician.

Ensure to drink more water.To keep your urine dilute This will help to control dysuria.

Further treatment will be decided by him based on findings of these tests and final diagnosis.

Today, Wednesday, February 25th, 2015, I have read your comprehensive reply to my question (sent to you yesterday), regarding possible BLADDER CANCER or alternately possible URINARY TRACT INFECTION.

I wish to thank you kindly for providing such thorough and extremely helpful medical assistance regarding this matter. Above all, I am deeply relieved to learn that I am probably NOT harbouring Bladder Cancer. As I understand from your reply, the medical condition which you have identified as being my problem, although urgent, is one which can be easily remedied with conventional medicine and with an obvious need for a rapid lifestyle/diet change as a Diabetic. (I will discuss more about this, in my next correspondence to you.)

So, with renewed confidence and with gratitude for your professional assistance, I will now be able to proceed to my specialists/doctor(s) with a relaxed spirit and armed with a printed copy of your medical recommendations, which you have provided for my condition, so far.

I should also mention that I am currently in the stellar care of my amazing Gynecologist who is proceeding against-the-clock with identifying some suspicious uterine cancer cells, before ushering me toward an urgently needed hysterectomy, within the ensuing weeks. This Uterine Cancer concern has suddenly trumped any other medical emergencies which might have come up recently. And, so you can understand why the additional worry of possibly having Bladder Cancer as well would have absolutely crushed my soul and spirit, at this time. In short, I thank you with all of my heart for removing that anxiety.
