To recapitulate: Male/56 - swelling on right side of scrotum - 2 weeks - consulted a Doctor - took antibiotic - felt like the testicle in the bottom of the scrotum - sore and hard - wants to know whether one testicle can drop -

With the history you have provided, this looks to be severe epididymo-orchitis on the right and due to inflammation and heaviness of the parts it may be sagging down.
I hope the following things are done, if not done, please get these done:
Clinical evaluation by an Urologist.
Urine: routine, microscopy, culture and sensitivity.
Tests of blood
Ultrasound of the whole abdomen, particularly for genito-urinary system.
High resolution ultrasound with color doppler examination of the scrotum
Antibiotic coverage and anti-inflammatory medications
Scrotal support.
Plenty of oral fluids
Further management as per the reports and clinical evaluation
