Suggest Treatment For Restlessness In An Infant

Question: Hi Doctor,

My daughter is approx. 7 weeks old.
For the past 2 days she seems a bit restless. She looks hungary frequently but, when we feed her she doesn't take much feed. She demands feed very frequently but, doesn't take much everytime.

She vomited last night while feeding. Same happened day before yesterday. What shall we do, please suggest?

I have gone through your query and understand your concerns. Let me emphasize on the fact that breast milk is the best possible milk for a baby and there can be no alternatives for the same. you need to do is to monitor the urine output. If the frequency is more than 6 times in 24 hours, you would know that the amount of feeding is adequate. If the amount of passage of urine is less or if the child is lethargic you would need to take the child to a doctor without delay.

You should burp the baby for at least 15-20 mins after each feed. Overfeeding or improper burping could lead to the child throwing up. All you need to do is to hold the baby head end elevated for this duration. The extra air taken causes distention and hence colics. Proper burping will ensure its escape and the milk will not regurgitate back from the stomach to the food pipe, thus providing relief. You can otherwise continue to give Coliaid when needed. You need not give it on regular basis. It is a safe medicine and you need not worry about any disadvantage of giving it.

Let me first ask whether the baby is exclusively breast fed or whether formula feeds are being given. Babies often get colics especially when on formula feeds. In such a case switching back to breast milk should provide relief.

Thanks for your reply.

Baby is given mix feed. Approx 60% is breast feed. But, sometimes when she sems hungary even after breast feed and no more milk is produced, we give her formula feed.

Specially during the night when baby gets up, we are generally giving formula feed.

Thanks for writing back. Let me emphasize on the fact that breast milk is the best possible milk for a baby and there can be no alternatives for the same. Most formula milks are cow milk based and hence difficult to digest. Moreover they make the child more prone to complications and infections. Mother's breast milk is produced on the demand of the baby. Hungrier the baby, more vigorous is the suck. More is the suck, more is the amount of breast milk produced. Giving formula automatically reduces the demand of the baby and thereby the production of breast milk. All you need to do is to switch back to exclusive breast feeding and the production will increase automatically. All you need to do is to monitor the urine output. If the frequency is more than 6 times in 24 hours, you would know that the amount of feeding is adequate.

Hi Doctor,
Thanks again for your reply.

I understand mothers milk is best but, she takes feed from one breast only and doesn't even try to suck the other one. She starts crying and turn her mouth away while we are trying to feed her. Thus feeding from one breast seems insufficient for her. Moreover, when she is sucking she goes to sleep and then wakes up after few minutes. So, this process in itself takes 30 min and then also we feel she is not full since she is not taking it effeciently.

My wife usually expresses the milk by hand or sometimes using pump and then feed her. The milk in the other breast where she doesn't latches on is mostly lesser compared to the other one. Pls suggest what best we can do in this scenario to avoid top feed.

Thanks for clarifying. I feel that there is some improper technique involved. It is possible that there is a structural difference between the two breasts. Please note that the entire brown area surrounding the nipples (the areola) needs to be put in the mouth of the baby. If there is a flat nipple, nipple shield can be used. Somebody needs to demonstrate the right techniques.

It is meaningless to express the breast milk. That would not serve the purpose.

The adequacy of the feeds is not to be judged from the behavior of the baby but from the frequency of passing urine.
