Brief Answer:
I suggest him to use a topical tretinoin cream

Detailed Answer:
Hello. Thank you for writing to us

I am dr.kakkar(dermatoologist). I have gone through your query and I have also reviewed the Image.

I can see a raised, red spot with few surrounding telangiectasias, which is probably due to skin atrophy due to long term steroid use.
It really seems like a non-inflammatory acne/ comedone lesion to me and steroid cream would certainly not resolve it.
I suggest him to use a topical retinoid e.g tretinoin 0.025% cream (Retin- A), once daily at night for a few weeks. Tretinoin is comedolytic and would help resolve it.

Regards

Tretinoin 0.025% cream

Detailed Answer:
Hi.

Yes, an acne lesion, specially if its non-inflammatory can last months, more so if its growth is arrested by steroid application.
Retin-A is worth a try.
