Suggest Treatment For Panic Attacks And Weakness While Having Vitamin D Deficiency

Question: Hi I am XXXX , My wife is suffering from GAD/HVS/Panic Attack , Blood Reports reveals she has Vitamin D deficiency , It is around 10. Doctor Has prescribed Xet-r 12.5 mg Morning and night along with Bio DS3. Petril MD .25 SOS for severe condition . She has left the medicine for 3-4 days and She is again looks to be attacked by Phobia /Panic and weakness on whole body . Does not feel like eating anything and talk to anyone ! Cries frequently, But this time no HVS or breathing problem ..
,How Long Should she take Xet R and Tables of Vit D..Is there any other medicine,suggestion can you advise pleaase ?

Thanks for writing to us. I can understand your concern for your wife.

In case of GAD/HVS/Panic Attack my first choice are also paroxetine (xet cr 12.5) and clonazepam (petril MD 0.25) sos. She has been prescribed right medicines and she improved also I guess.

For vitamin D deficiency I used to prescribe vitamin D chewable tablets 60000 IU per week for 12 weeks. This supplement can be stopped after deficiency improves.

Paroxetine is and SSRI group of drug and sudden stopping of the same can cause drug discontinuation symptoms if it has been taken for more than 8 weeks. She may have such symptoms likely as she has not taken medicines for 3-4 days.

Generally after complete improvement medicines can be stopped gradually over 3-6 months depending upon history and symptoms severity. I used to apply cognitive behavior therapy (CBT) simultaneously after initial stability. CBT can help to cure the disorder or prevent relapse and person can stop medicines easily.

I recommend restarting medicines and consult her psychiatrist for reduction in medicines slowly.

Also let me know since when she is suffering from this and since when medicines are going on and how much improvement was there?

Greeting Dr. Solanki , Thanks for your prompt response , I have asked her to start the prescribed medicine .

My follow up question is : She worries or thinks too much about any situation for an example , my daughter if she does not do well in school , It will be one of the cause for her to start thinking she will revolve her mind to 360 degree with continuous thinking of her , what my daughter will do ,my husband is paying hefty money for education , she is not doing well kind of , Although I ask her to ignore and move on daughter will improve gradually ! I am paying school fee I have not tension then why you are worrying too much ..!! Nothing helps ..no counselling works .. I can change her DNA!!. It is on of the example there are many ...Even a small scuffle leads to a undesirable situation which I call Severe HVS.. I need to remain calm to avoid any such situation ..I even forgot when I last got angry on her now .

How can her habit of thinking /worrying and moving mind at 360 degree be changed .

stopping medicine /taking medicine is not a problem , but root cause lies somewhere else ..Can you advise how the root of the problem can be treated ?

You can understand your wife very well that is positive part. By being calm you are providing a great help to her. Whatever you have narrated about her symptoms are typical characteristics of patients with GAD.

Medicines can control it very well. But to cure it we need to deal with it at root level. Yes we cannot change DNA but we can change her thinking process with help of herself. In that CBT and psycho dynamic psychotherapy can help. I have treated many of the patients. Therapy to be successful first need is readiness of the patient that I want to change my thinking pattern and some psychological mindedness (thinking about thinking). Second important aspect is skill of therapist. Psychotherapy does not mean simple counseling.

CBT is specifically used to correct automatic thoughts and distorted cognition. In CBT psychiatrist/psychologist helps person to identify his/her automatic thoughts and faulty behavior which would ultimately be modified by person with active efforts and help of therapist. It also includes time management, daily sleep, meals and exercise timing.It may require at least 10 -12 sessions each of 45-60 minutes depending upon history.

In psycho dynamic psychotherapy detail history from childhood is obtained and after initial psychoanalysis some intervention is done to change psycho-dynamic.
