Brief Answer:
As discussed in details below.

Detailed Answer:
Hi.
Thanks for your query.
To recapitulate: Male/60 - dermatitis - Econazole nitrate - told not to use any other topicals for hemorrhoids - lot of discomfort and pain - wants pain medication to relieve discomfort from hemorrhoids...
Since there is discomfort or pain in the anal area, this can be due to infection or inflammation secondary to dermatitis.
Hemorrhoids usually do not cause discomfort or pain unless complicated by thrombosis and/or infection.
Hence you would require an antibiotic and an anti-inflammatory medication like Ibuprofen if it suits you.
You can certainly take Ibuprofen or such medication that is OTC and suits you as you might have taken before.
Antibiotic being a prescription medication needs a prescription of your Doctor for the Pharmacy to honor it.
Also take a mild laxative to keep the stool soft enough not to cause any pain while passing a bowel movement.
Sitz bath in a tub with warm water followed by making the area dry by cotton cloths/ paper napkins or blowing the air from hair dryer.
Take multivitamins and probiotic as it helps recovery from Dermatitis.
Consult a General Surgeon and/or a Dermatologist to make sure the diagnosis and have proper additional treatment as may be needed as per the findings.

I hope this answer helps you, please feel free to ask for further relevant queries if you feel that there is a gap of communication.
