Brief Answer:
Need examination and possible imaging study

Detailed Answer:
Thank you for your question. Great to hear that you are so actively engaged in exercise activity. I find it curious that you have no pain at all while lifting your weights but yet lying down produces problems.

My suspicion is that you may have a small amount of calcification due to arthritic degeneration either in the shoulder or the bottom most portions of the left neck which upon being compressed or irritated for a couple of hours while in the supine position may be causing your problems. The headache that seems associated is also on the same side as the painful left shoulder and so that would suggest a relationship between those 2 things.

What I believe will adequately sort things out very nicely would be a good examination by an orthopod or PM&R doctor who can look at the full range of motion of your shoulder and neck both in the upright as well as supine positions and that either some plain films of the left shoulder and neck or CT scan of that area may reveal arthritic disease and changes that may be afoot.
