Suggest Treatment For Non Productive Night Time Cough

Question: Pt c/o complaining of a nighttime cough 3-4x per week, non-productive cough daily with occasional wheezing. Soc hx: Smokes 1 ppd x 5 years. Fhx: father and one brother have asthma. On exam all is normal except you hear an occasional expiratory wheeze.

What will be the diagnosis and what tests and medicationd needed for this pt?

It may be due to post-nasal drip(PND).

Detailed Answer:

It may be due to post-nasal drip (PND).
It occurs when there is excessive mucus production by nasal mucosa.Excess mucus accumulates in the throat or back of the nose.
It may be caused by rhinitis,sinusitis,gastroesophageal reflux disease (GERD),esophageal motility disorder etc.

Patient need proper clinical examination and investigations.
Patient should be examined by a ENT specialist.Allergy cause should be ruled out.
Gastroesophageal reflux disease (GERD) and esophageal motility disorder should also be ruled out as it may cause similar symptoms.

Patient may need routine hemogram,allergy tests,Upper GI endoscopy.Further tests can be done if needed.

Proper treatment depend upon underlying etiology.
Patient may need antihistaminics,decongestants,bronchodilators,nasal steroid spray etc.
If GERD is confirmed then proton pump inhibitors,prokinetics should be prescribed.
