Question: Ive been sick for the past couple of weeks. It started with nausia, vomiting, low fever and difficulty urinating. I had white blood cells in urine but negative cukture. I took Cipro for 5 days and did start feeling a little better. However, three days later I have the urge to see constantly and but can barely pee. They put me on bactrin and it's not helping. I have to strain really hard to get anything to come out when I pee and feel like I have to pee constabtky. It feels like something is spasming I also have urine leakage and a terribke headache. I had a CT of my stomach and abdoman and everything looked normal.

Brief Answer:
Could be either due to Over Active Bladder or narrowing of Urethra

Thanks for query .
Based on the facts that and relevant tests reports that you have posted which are normal the sensation of urge to pass urine with pain in lower abdomen that you have is most likely to be either due to
1) Over Active Bladder (OAB) syndrome or
2) Narrowing of Urethral Passage ( Stricture)

Under normal circumstances one gets sensation to pass urine when bladder gets filled up with 250 -300 ml of urine however in OAB there are involuntary contractions of bladder (spasm) of the bladder even after it gets filled with small quantity (25-30)ml giving rise to get sensation to pass urine (URGE).

The second possibility is narrowing of urethral passage (Stricture) secondary to UTI.

I would suggest you to start taking anti Cholinergic drug like Darifenacin once daily .It would take a week or 10 days to observe the noticeable response and will have to continue taking drugs for 2 months to resolve the issue completely .

However in case if you do not find any improvement in symptom of urge after 10 days of treatment you will need to consult qualified Urologist for assessment and endoscopic examination of urethral passage and bladder (Cystoscopy) He would do calibration of urethra (Dilatation) simultaneously during cystoscopy .This would widen (Dilate ) the urethral passage and help in compete emptying of bladder during urination and there by reducing the symptom of urge.

Hope I have answered your query ,please feel free to ask if you have more questions,I shall be happy to help you
Thanks and Regards.
