Suggest Treatment For Lump On The Ankles

default
Posted on Thu, 12 Mar 2015
Question: I have a lump on the inside of my right leg a few inches above the ankle. It is very hard and sore, I first noticed it when I wore socks that came to my calf. I had to fold them down to my ankles. I thought at first I just bruised it working outside. But was over 6 weeks ago. It is still hard and tender, although my skin is not as red as it was. Could it be a blood clot or something more serious?

Providing clinical photograph will be helpful

Detailed Answer:
Hi
Welcome to Healthcaremagic
Greetings of the day
Dear Ms

From the short description of lump and soreness around the ankle ,ppossibility of thrombosed vein needs to be considered. Exact diagnosis will depend on clinical examination.
It would be very helpful if you could provide me with the photographs of the lesion for further assessment. Also kindly provide me with the following additional information

1. Where is the exact location

2. Is it associated with itching or discharge ?

3. Is the lesion very much elevated above the level of skin ?

4. Is it painful to touch ?

5. Do you have any history of trauma to the site ?

6. Do you have any significant medical condition like Diabetis /HHypertension

1) The exact location is approx. 5-6 inches above the ankle bone.
2) There is no discharge or itching. The lump is approx 2" or more around inside my leg, there is no skin breakage.
3) Yes, there is a slight elevation, there is no outside lesion. That's why at first I thought had a bruise, no cut, scratch no mark of any kind outside.
4) Not extremely painful, more tender. However, it hurts when I place pressure on it. That's why the new socks hurt. They were tight around the calf.
5) I got kicked by a horse as a child. It left a half moon mark on my leg. This bruise is slightly below and to the right of the kick mark. The kick mark left a permanent scar approx 2-3" and a slight permanent indention.
6) I do not have hypertension nor diabetes. I do have Lupus, also episcleritis (eyes) So I take Predisome for both. Also I have arthritis, taking Celebrex and Methotrexate, Advil PM.
Taking a picture would really not help much. Some of the redness is gone and the elevated part does not show up in pictures. There is no outside mark on my leg, the hard lump in inside the leg and does not show up in a picture. I tried many angles, but no significant part can be seen.

Brief Answer:
Likely to be Panniculitis

Detailed Answer:
Dear Ms

Going by description of your symptoms possibly of Panniculitis (inflammation of subcutaneous fat ) needs to be considered, it's also more likely seen in individual with Lupus.

There is no cause for concern as it's self limiting. I would suggest you the following

1. Adequate rest and elevation of limb while sleeping (place the leg on two pillows while sleeping )

2. Local application of Ice will be helpful

3. As you have taken steroid (Prednisone ) that will also help reduce inflammation

4. Analgesic like Ibuprofen will be helpful

Do get back to me if you need any further assistance, will be glad to assist you.

Wishing you and your family a very happy and healthy life.

Take care

Best regards
Dr Deepak

Thanks, Doctor. I have 2 different Predisone mg. One is 5 mg, I take daily unless my epicleritis comes on then I have a 20 mg I take. If I take the 20mg at the beginning of the redness or if a headache and pain directly behind my eyes (inflammation starting) the 20mg knocks it out in a day or too. But I skip the 5 mg for a couple of days. I'm back to normal and back to the 5mg for lupus.

Not much to do outside, even here in XXXXXXX Guess I'll read more with my leg propped up. See what happens. If not better in a couple of weeks will see my fam. phy.

Again, thanks XXXXXXX
