Question: My husband had a fall off his bike and has left side rib pain and blood in urine. Should he go to the ER? Also on blood thinners

yes you should go right away!
Blood in the urine (particularly if it's visible) denotes urinary tract bleeding which - unless your medical history suggests otherwise - can originate from the kidney. Kidney bleeding can be very serious and needs to be confirmed or excluded as soon as possible!
Also the left rib pain may mean that his spleen could be injured as well. This is serious as well.
Blood thinners make things worse because a minor trauma can bleed for so long that it causes serious trouble.
Checking his blood pressure and pulse rate can give an indication of significant blood loss (low blood pressure and increased pulse rate).
