Suggest Treatment For Itchy And Swollen Testicles

Question: Hello,

I have been having a rather debilitating issue going on since early 2013. Let me preface by saying I am an alcoholic and have been a heavy drinker for 16 years. I am now 36. In early 2013 I was about 6 months sober and in a halfway house. I woke up one random morning and my scrotum had swollen up. It was not painful but very uncomfortable and swollen and I had a high fever and the chills and felt very tired. Ever since that day, I have been having ongoing issues with my scrotum. It will often swell and after it swells it gets EXTREMELY ITCHY. The swelling is every once in a while but the itching is almost every day and is debilitating. The itching is so bad that it often prevents me from falling asleep because I can't stop scratching long enough to fall asleep. It is also preventing me from getting a job because I can't be constantly scratching myself "down there". The best way I can describe this is like mental torture. I have to scratch so much and itch so much it actually drives me crazy and is making me depressed. I have been to a Dermatologist several times and a urologist twice and nobody knows exactly what is the problem. When I look at the scrotum in the mirror, it is very red and inflamed and has large raised red bumps on it. I should mention I had Cancer (Leukemia) as a child and had a testicular relapse and had to have severe radiation to the testicles which now makes it so I no longer produce my own testosterone (I am on testosterone replacement medication) and I can not father a child due to the damage to this area. Things that seem to at least temporarily help the itching are frequent showers, periods of not drinking, and better eating habits. I need to also mention that I am pre-diabetic so that is a big issue with the drinking too. I have not idea what is causing this and it is so frustrating and nobody seems to be able to help me. Do you have any suggestions as to what may be causing this?

Your scrotum seems to be indurated,erythematous,enlarged and edematous.

Radiation treated testicles are more prone to develop frequent relapses and are difficult to manage.Radiation treated organs are known for their detrimental side effects in the long run.

I advise you to have colour doppler ultrasound of your testis to document the testicular inflammation with possible eye on leukemic mets if any before the empirical therapy.

Meanwhile you need antibiotics-antibacterial +antifungal(in view of pre diabetic and chronic alcoholic state) to control the infection in the form of tab azithromycin and tab itraconazole for 7-14 days.This can effectively alleviate all your symptoms.

Note:These drugs have to be taken only under strict medical supervision
