Suggest Treatment For Hip Pain While Having Trochanteric Bursitis

default
Posted on Tue, 21 Jun 2016
Question: At age 66, I've been in physical therapy 3x/week for 4 weeks for hip pain - diagnosis is trochanteric bursitis. However, though therapy results in some relief, I still have pain in hip and quads on left side, discomfort in walking, less pain in the morning but increasing pain during the day. By night, weakness and pain radiating and ever present. PT orders have run out, but improvement only at about 30%. No x-rays were ordered in the initial exam. Otherwise in excellent health. Should I go back and request X-rays? Is their a possibility that the original diagnosis is incorrect?

Brief Answer:
Arthritis, gluteal muscle/tendon injury have to be ruled out

Detailed Answer:
Hello,
Thank you for your query.

I'm Dr. Shuba Hariprasad and will be answering your query today.

A diagnosis of trochanteric bursitis can be made based on clinical examination and findings.

However, other causes of musculoskeletal origin (gluteal muscles/tendon injury, arthritis) need to be ruled out as the cause, especially since therapy has not helped much.

An X-ray will show bone defects and an MRI will show any muscular, tendon or bony injury.

I would :
- clinically examine you, check for range of movements, flat feet I addition to a complete physical exam

-do an MRI of your hip and thigh.

Based on this I would advice a treatment plan.
If conservative and physical therapy don't help, surgery may be required to remove the bursa or repair the Injury. This can be done arthroscopically in most cases.

I suggest you speak to your orthopedic Doctor and request for imaging studies (mention that therapy hasn't helped much).
Hope I've answered your query and this helps.

Please let me know if there is anything else I can help you with.

Wish you good health.

Regards,
Dr. Shuba Hariprasad
