Question: hello. Im a 37 year old female about 5'3 and 124lbs. last monday i ate a bad hamburger, and was sick for about 2 days and i was fine after that. This monday i got really dizzy, flush, and nauseated. I went to doc and i ended up having sinus infection but the nausea didn't stop. I ended up going to the ER on thursday and they said it was acid reflux. Im taking the meds, pepcid 40mg, for that but getting very little relief. They also gave me zopharn. So now the nausea is not as bad, but still feel the burning sensation rising and falling. I have also lost 9lbs with in a week and haven't eaten since sunday. Any advice

Brief Answer:
It could be dyspepsia and not just acid peptic problem.

Detailed Answer:

Hello,

I have gone very carefully through your query.
Our gastrointestinal tract has a forward propulsive movement (peristalsis) that moves the food material forward as it is digested, absorbed and finally expelled as feces. If somehow this movement gets hampered or there is a infection in gut, food along with the digestive juices would stagnate, stomach acid will reflux into the food pipe.Degradation of undigested food in the intestines by the intestinal bacteria would lead to the contamination in the particular area of intestine. This would cause severe swelling in intestine causing nausea and other symptoms.
I would suggest you following tests-
Ultra sound abdomen
Stool test under microscope
Complete blood count
Total and differential leucocyte count.

If stool test is positive for bacterial or protozoal infection.Start taking Zithromax(Azithromycin 500 mg once daily) and secnidazole 1 gram daily for 5 days.

As of now to control burning sensation pepcid 40 is a bit low dose.I would suggest you to add Levosulpiride. Itis a prokinetic drug and is very effective in controlling reflux of acid back into esophagus.A better way is to take it with Rabeprazole trials have shown that its very effective when combined with Rabeprazole. So you can switch over to Rabeprazole from pepcid.
Add a probiotic like VSl#3 twice daily to replete the gut flora.
General precautions are-
Follow a bland diet (low in fat, not spicy)
Eat frequent, small meals.
Avoid the use of alcohol, tobacco, and caffeine.
Do not eat food less than 2 hours before bedtime.
Raise the head of the bed before sleeping.
Avoid clearing of the throat.
Zofran can be taken as and when required.

Ok. I will make an appt to see a gastrologist. Also I had an ultrasound but he only checked the gallbladder. my white blood cells were up but he said it may have come from the steroid shot I received for the sinus infection

Brief Answer:
Follow up.

Detailed Answer:
Hello again sir,
white blood cells are of 5 types.
Some are raised in acute condition while others in chronic condition.Steroid shot i dont think has a relation to it.
For me it can be due to infection in gut which is still persisting.
USG gall bladder won't review complete abdomen.
Hope you were helped by my answer.
Suggestion to improve it are welcome.
