Suggest Treatment For Cough And Wheezing In A Child

Question: My child of 11 years has wheezing when she gets a cough and has a SPO2 of 91. Doctor has given Budecorte and Levolin for nebulisation. Is the diagnosis correct or do we need to do more tests to confirm her exact problem?

Wheezing in a child could occur due to respiratory tract infection or due to childhood asthma. Whatever be the cause, your doctor has rightly managed the case by giving Budecort and Levolin nebulisation which is just the thing required.

A chest X ray and blood count will help in identifying the underlying cause.

What tests are to be done to detect the exact reason for
1) Wheezing. Everybody who has a cold or cough doesnot have wheezing.
2) Fall in SPO2 and increase of heart rate.
3) She is alergic to dust which sometimes trigger sneezing and wheezing.

What should be the long term treatment philosophy, just symptomatically treat with nebulisation or do tests to identify the cause and treat the reason and what are the steps to do that.

Could be due to a viral infection

1. Wheezing occurs due to constiction of the respiratory airways. The default assumption would be a viral cause for which no testing needs to be done. A blood count should yield a normal result. A diagnosis of asthma is not usually not made before 5 years of age but can be made based on frequency of recurrence and the pattern of presentation. No testing is usually done but pulmonary function test would have been ideally needed to confirm.
2. Fall in SPO2 and increase in heart rate could indicate respiratory distress calling for medical attention. It is more infavor of respiratory tract infection.
3. Allergic component is commonly associated with viral infections.
