Suggest Treatment For Burning Sensation And Pain In Testicles
Question: I have been diagnosed with urinary gonorrhea infection (burning pain in the testicles and urethra) after unprotected intercourse with a new partner. As a first line treatment, my doctor injected 240mg gentamicin intramuscularly, together with 2g Azythromycin (taken orally). The symptoms disappeared almost completely for 3 days. On the third day, however, they returned, and the pain was much worse after that. As a second line treatment, the doctor injected 250mg ceftriaxone plus 2g Azythromycin, but without much effect. As a third line treatment, I have been given 360mg gemifloxacin orally plus Doxycylin (2X 100mg per day for 1 week). Again, there is no improvement to the symptoms. I live in the Philippines, where the incidence of antibiotic resistance in Gonorrhea seems very high.
What alternative treatments can I try now? Are there any antibiotics similar to gentamicin that are worth trying?

Thanks for your query ,based on the facts that you have posted it appears that you have been diagnosed to have Gonorrheal infection following unprotected sex with new partner .
I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Was culture and sensitivity test done prior to starting antibiotics.?
Please get your urine culture and culture of a urethral discharge done to find out sensitivity pattern of the organisms ,
Organisms (Gonococci)which were earlier highly sensitive to Erythromycin and Doxicycline have become resistant in recent times .Hence it is suggested to do culture and sensitivity to know the sensitivity pattern of the organisms so that one can take appropriate antibiotics to combat infection .
Normally report of culture and sensitivity is available within 72 hours till then you can continue with Erthromycin with Inj Amikacin twice daily .Later on switch on to appropriate antibiotics as per culture report .
