Suggest Treatment For Blurred Vision After Cataract Surgery

Question: Chief complaints
This is 86 years old male member who presented with blurred left vision followed by left cataract surgery with post-operative intraocular injection for 18 months ago.

On Examination
     Unaided distance vision: Right Eye 0.63 LogMar; Left Eye 1.50 Log Mar
     Current glasses: Right Eye DS (0.70 LogMar); Left Eye DS (ni LogMar)
     Right Eye: Pseudophakic
     Left Eye: Pseudophakic bullous keratopathy. Slight peaked pupil with iris suture
     Intraocular Pressure: Right Eye 9; left Eye 7 mmHg Pentacam

Past Medical/Surgical History
He has a history of left eye cataract surgery

Diagnosis
Bullous keratopathy

kindly advise which of the following procedures is much advised for the above case and why
Descemet’s Stripping Automated Endothelial Keratoplasty or,
Corneal Transplantation

The treatment for Bullous keratopathy is by transplantation of the cornea to replace the damaged cornea by a healthy donor cornea.
Descemet's strippling endothelial keratoplasty is one of the recent types of surgery. It is a sub type of corneal transplantation in which only the innermost layer of the cornea called as endothelium is tranplanted.
It has got many advantages over traditional corneal transplantation like decreased rate of rejection, minimal astigmatism following surgery....
This can be done only in early cases of bullous keratopathy in which only corneal endothelium is damaged.

By seeing the vision in the affected eye, I think it is in the early sages and can be treated by Descemet’s Stripping Automated Endothelial Keratoplasty.
Hence I would like to advice you to go for Descemet’s Stripping Automated Endothelial Keratoplasty.
