Question: My scrotum is engorged. Approximately twice? Maybe 1.5 times it's normal size. Should I go to the emergency room?

Please find my observations below.

1. A sudden enlargement of the scrotum to twice its size can be considered an emergency and you should consult the emergency room.

2. It is possible that there is a torsion due to compromised blood supply or an infection. However most cases of torsion or infection will cause pain and you have not mentioned it. Rarely some of the conditions might occur without pain to start with.

3. The doctor will examine you and probably recommend the ultrasound scan with Doppler to know the changes in the scrotum and blood supply.
