Question: Having had two TURPS I have had recurrent urinary tract infections. The treatment has been only antibiotics
which cures the problem until it recurs. I have undergone several tests none of which have revealed a cause.
A recent urinalysis did not reveal any infection. However I experience a burning sensation in the head of my penis when urinating and occasionally spasms when I am not. My urine is cloudy and stinks.
What to do?

Hi,
You have outlined symptoms of a fairly common problem. Several patients with prostatic disease do experience symptoms in pelvis which are not necessarily due to infection, though antibiotics do help. If your urine flow is good, then there is no reason to be alarmed. You can take a long term low dose antibiotic such as doxycycline 100mg once at bedtime, and drugs like flavoxate at 200mg thrice daily could mildly reduce bladder spasms, while stronger drugs like vesicare might be needed if spasms are severe. You can try alkalinizing urine with potassium citrate syrup at 10-15ml thrice daily with water after meals and see if you are comfortable. Symptoms could take as long as 3-6 months to resolve.
If the urine flow is not good, you might need tests like retrograde urethrogram to know if you have any stricture if urethra or a contracture of bladder neck following TURP, though a uroflowmetry is a good enough simple test which might give a good indication of what is happening to your urethra and prostate.
