Brief Answer:
Acne Rosacea or Atopic Dermatitis

Detailed Answer:
Hi, welcome to HCM,
Being a dermatologist two differential diagnosis comes in my mind, Atopic Dermatitis and Acne Rosacea.
Acne Rosacea is a chronic disease and persistent redness is seen over forehead, nose and chin. blood vessels look enlarged and tiny acne like papules are also seen.

Atopic dermatitis presents with redness, scales, crack, crust and vesicle/papules. Atopic dermatitis also involves flexure of hand and knee joint. some times it also remains associated with asthma.
