Question: I was awakened by a sudden excruciating shoulder muscle spasm, pain is very local to one of the muscles in the deltoid and deeper into the joint, I can make it go away if I move the arm in certain positions that contract the muscles do they are not stretched or exerting, moving in some directions causes the pain


Pain is goes from an 8 to 2 if moved into a relaxed position.

This serious muscle spasm can be caused by many things. It may be from overuse of the muscle or from mild dehydration or an improper balance of minerals and electrolytes in the blood. Damage to the shoulder joint can also cause it.
Muscle relaxers like flexeril and anti-inflammatories like advil can help as well ans what you have discovered about changing positions. It is also important for you to make sure you are drinking plenty of water to avoid dehydration.
Taking a multivitamin/multimineral daily may also help avoid this in the future.
If the pain is severe and not resolving you need to see a doctor and may need an xray of the shoulder to rule out arthritis or damage to the joint.
Warm compresses and physical therapy can also be very helpful.
