Posted on Sat, 25 Jun 2016
Question: Hello Doctor,
I request your advice on couple of problems:

1. I have pimples over my entire back and shoulder throughout the year. The pimples are very rare on face and other parts of the body. But the back and shoulder has it throughout the year.
2. Other problem in line with this is in winters (from Nov-Feb) my feet stink a lot. There are pores that come on my feet below and the skin turns white. It is a very foul smell once i remove my shoes. Surprisingly this problem occurs in winters only.

Request your expert advice.

Brief Answer:
Body acne, plantar hyperhidrosis and pitted keratolysis

Detailed Answer:
Hello. Thank you for writing to us

I am dr.kakkar(dermatologist). I have vone through your query and i have noted your concern.

Acne can affect back and shoulders just like face. All these sites are rich in sebaceous glands. I suggest you to use a benxoyl peroxide body wash e.g brevoxyl creamy wash instead of a soap for bath. In addition you may also take a tablet of doxcycline 100mg twice daily for 2 weeks.

As far as foul smell from feet is concerned and pits on soles, this condition is due to excess plantar sweating and bacterial overgrowth i.e plantar hyperhidrosis with pitted keratolysis. I suggest you to use Aldry lotion (aluminium hexahydrate) for undersurface of soles, once daily at night. This would act by reducing sweating as well as inhibit bacterial overgrowth. However if this proves ineffective then the most definitive treatment would be intradermal injections of botulinum toxin to inhibit sweating from soles.

Hello Doctor,

Many thanks for your detailed answer. I have started on the prescribed medication.

My follow-up question is - my neck both at the front and back is relatively black in colour as compared to the skin colour of the body. Can something help get the neck complexion to match the skin complexion of the remaining body. Thank you.
