Question: New symptoms within last 2 weeks - constant headache with nausea and some vomiting, fatigue, higher heart rate than normal - usual resting heart rate is 60-75. Now resting heart rate is usually in the 90s. BP was 100/74 typically. Lab results as of yesterday: TSH 4.24 T3 1.04 Sufficient levels per lab: TSH 0.37-4.68 T4 0.78-2.19 Current levothyroxine dosage 50 mcg Should my medication be adjusted? Are current symptoms related to thyroid/pituitary?

I have gone carefully through your concern.
The symptoms you described are not related to thyroid pathology as your hormonal level is within the normal range under 50 mcg of levothyroxine nor should the dose increased.
Regarding the possible pathologies that may cause it, of course a detailed clinical examination and history is needed as well as some biochemical tests to exclude some common pathologies such as liver or gall bladder pathologies, renal pathologies, electrolyte disturbances, neurological disorders ect.
An adrenal insufficiency may also be the cause of nausea and vomiting but other more common pathologies should be excluded before.
