Question: My son was seen at a pediatric urgent care for purulent drainage from one of his ears with suspicion of an ear infection. He has ear tubes in both ears which were put in about 8 months ago. The practitioner prescribed augmentin, but I wanted a second opinion on the treatment she recommended because he is already having gastrointestinal issues that I do not want to exacerbate if he can be treated with antibiotic ear drops instead.

Hii. Your son is have ear discharge and he is also having ear tubes.These ear tubes are to be put in ear if there is any fluid accumulation in the ear and to maintain the pressure in the middle ear. This condition is known serious otitis media. There is always chance of watery ear discharge after putting the tubes as the fluid gets drained. But if the discharge is thick, yellowish, foul smelling then it's evident some bacterial infection has taken place place. So it is always needed to start antibiotic. Augmentin is the first line drug for young and old patients and it acts very efficiently in this type of disease. You can start higher antibiotic like cephalosporin but only if augmentin cannot resolve the infection. Cephalosporin and other antibiotic need to be reserved for future if treatment with augmentin fails. Otherwise child will develop resistance easily with antibiotic in future. So continue augmentin for at least 10 days and wait for the result. This serous otitis media in child is almost always due to adenoid enlargement at the nasopharynx or due to any allergic rhinitis. So do a x ray paranasal sinus and x ray soft tissue neck lateral view to see adenoids. Start some mometasone or fluticasone nasal spray to reduce the adenoid. Take steam inhalation. Start some antiallergic tablet levocetrizine also. Tell child to blow balloon intermittently and chew chewing gum. You can continue ciplox ear drops also to reduce the infection. If due to augmentin child is facing some gastrointestinal issues then you can start lanzol kid junior to reduce the acidity. Please review if you have any queries.

Why would augmentin be tried before the drops with a child that has tympanostomy tubes in place? Have the guidelines changed? Sorry I am just trying to understand the rationale so I can avoid using systemic antibiotics if they aren’t necessary. Thank you

I will suggest you to start with oral augmentin if there is mucopurulent discharge from ears from tubes along nasal congestion with nasal drop and spray. Rest depends on you.
