Brief Answer:
Deep vein thrombosis possible.

Detailed Answer:
Hello dear, welcome to Ask a doctor service.
I read your query and here is my advice.

Sometimes the swelling after the operation can last for 3 to 6 months.
This can be normal recovery swelling.
If the recovering has finished and swelling gone and now starting again and with pain it can be dip vein thrombosis issue.
In this case he may need to do an ultrasound of the leg veins.
Also he may need to do a blood test called d dimer.
Using blood thinners in this case can help.
Heparin is given intravenous or enoxiparin or fraxiparin . Seek medical attention soon.
Hope I have answered the question.
Let me know if I can assist you further.
