Question: My father aged 79 yrs having Dilated cardiomyopathy (DCMP) with severe lv dysfunction has got his right femur bone fractured.

Thanks for the reply, but I request you to please reply my pointwise queries in detail. I'm quoting my detailed question again.

My father aged 79 has got his right side femur bone(thighbone) fractured on 27th. He was taken to the Orthopedician who after check up suggested consultation with Nefrologiat, Cardiologist, Anesthesiologist and Physician for deciding on surgery.
Here is medical history of my father -
My father has already got his left femur bone fractured and operated in 2010.

He has been getting treatment for some weak cardio, enlarged prostate and some problems with urination since 2011.

We got all pre operative tests done and Doctors highlighted followings-

- Weak heart pumping, as low as 30% only
- Createnine level 2.1
- All other parameters were OK.

On the basis of above Doctor ruled out possibility of surgery due to high risk of cardiac performance. He has applied traction pin with Bohlers kit and suggested traction with back lift excercise and to let the bone join itself. He has told that bone may join in 2-3 months and my father will be able to walk with support after that.
Now my father is bed rideen with the applied traction. Doctor has asked to re check the joint by X-ray after 3 weeks. My father is taking following medicines in total for fracture, cardiac issues, Prostate and high Createnine levels -

*Morning:*
Cardivas 3.125 - 1/2 tab
Dytor-10
Avas-10
Sartel AM
Dolo-650
CCBM-Plus
Ciplox-1000

*Aftenoon:*
Dolo-650

*Evening:*
Dolo-650
CCBM-Plus
Cardivas 3.125
Silodal 4

Everything seems to be alright but my father is feeling heavily sedated during daytime and in night he is uncomfortable due to being awake.

I want to get your advice on these points -

** How much chance is there of joining the bones at this age without surgery ?

** How much time will it take to heal and join the bone?

** Will he be able to walk again after healing?

** Is there any serious reason of his excessive sleep during daytime or is it effect of medicines only? If there are other reasons, what shall we do?

Hello.
Thanks for writing again.
1. At his age the chances of joining the bone with rest and spontaneous healing is nearly 50-60%. In some cases some bone fragments remain ununited ( non union) and person may walk with the help of a walker.
2. Usually it takes 3 -4 months for the bone to heal properly along with a good diet, regular physiotherapy and calcium supplements.
3. After healing if all the fragments join properly then he can walk properly.
4. The excessive sleepiness during the day will cause insomnia at night. You can get a mild antianxiety medicine prescribed by his treating physician for this.

My father was tested for blood urea and Sodium on 8th and results were as below-
Blood Urea - 83 mg/dl
Serum Creatanine - 1.56 mg/dl
Serum Sodium - 125 mmol/L
Serum Potassium - 3.68 mmol/L

Are above values abnormal, if yes then can it be treated with some medications? Is this low level of Sodium causing the excessive sleepiness? Reports are attached for your perusal.

Please advice.

Needs an indoor treatment under physician / cardiologist.

Detailed Answer:
The excessive sleepiness can be due to low levels of sodium as well as due to raised urea levels. He needs a strict care of a physician to regulate these levels for which it is best to consult his cardiologist. Giving some medications at home will not help as these levels need to be monitored regularly.
