My 78 Your Old Brother Had A Stent Put In

Question: My 78 your old brother had a stent put in his right middle cerebral artery earlier this year. Today his cerebral angiogram shows a 75% stenosis. He is asymptomatic and had the angiogram to be cleared for a bicepts tendon surgery. What is next if the stent is closing?

Intracranial angioplasty and stenting are increasingly being used to treat patients with recurrent cerebral ischemia due to intracranial atherosclerosis, which is a cause of 8-10% of all ischemic strokes.

A middle cerebral artery (MCA) stroke occurs when the middle cerebral artery, which supplies the frontal lobe and lateral surface of the temporal and parietal lobes with blood, becomes blocked.

Your brothe's actual angiogram show 75%stenosis.
This means that it is very important to take regularly the antitrombotic/antiplalets and statin treatement in order to prevent the total stenosis of MCA.

Apart angioplasty to reopen the artery with stent , cerebral artery bypass surgery can be considerated.

II want to underline that ,if you see neurological deficits ,it is very important to go to the ER.
In the acute setting, the interventions for an MCA stroke are IV tissue plasminogen activator (TPA) and thrombectomy, if the patient qualifies. As mentioned above, the time of onset will determine if the patient qualifies for these interventions.
