Question: I’ve been on birth control since I was 15 and I am now 21. Within the last year I’ve started bleeding for 12+ days even while still taking the pill. What is coming out during those 12+ days is brown clotted blood rather than fresh blood and I also get extremely painful cramps. Is this something I should be concerned with?

1) Reason for such bleeding / spotting could be due to hormonal variation at your age and switching of pills.

2) Other reasons could be polyp / fibroid in uterus. Adenomyosis or endometriosis could be reasons.

3) I suggest see your gynecologist once and see if you can request for an ultrasound scan to check the above plans if exists.
