Is Laser Surgery Or Latanoprost Drop More Effective At Treating Glaucoma?

Question: 77 y/o male.No symptoms. No family history of glaucoma. I was diagnosed with glaucoma in right eye and increased pressure in left eye (33/25).
Have some damage to optic nerve cup on exam with some decrease in visual field in right eye.

I saw two doctors. One suggested going straight to laser surgery first, the second suggested
i start using Latanoprost drops in both eyes because he said laser surgery would not sufficiently lower the pressure enough in my right eye. (only lower it 20%) And lasers only work temporarily.

Which do you recommend? Would laser followed by drops, or drops followed by laser if needed be best? Any downside to either fist?

Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

For glaucoma there is only eye drops, tablet and surgery is available option is to be decided by treating doctor. If pressure is very high then it can be lowered by tablet.For surgery trabecule is recomanded. Trabecular mesh-work is incised and channel is opened up.It can be supported by tenalol drop. Followup is to be made by taking repeated IOT. If it is higher then can be controlled by available drops, tablet or surgery. Fundus examination is to be done and cup disc ratio to be noted.In your case it is altered. So the type of glaucoma can be analysed by slit lamp or gonioscopy. Accordingly medication can be recomanded. If damage is done, with proper treatment it is reversible so better to start treatment early. Field tracing should be done if scotoma detected,then immunologist can be consulted and thyroid function can be done.
