Is An Abdominal Cerclage Appropriate For Treating Bicornuate Uterus During Pregnancy?

Question: My wife is currently carrying and is in 16th week. She is having Bi-coronate uterus and dr. has advised for Cervical stich. From internet I can understand this can be done through Vagina or Abdomen. Which option is recommended? Are there any side effects for this?

Thanks for the query to hcm,
In a bircornuate uterus, if the pregnancy is already 16 weeks old and growing well then the next concern remains that there may be a preterm delivery due to possible cervical incompetency and hence cervical encerclage is advise with an aim to continue the pregnancy till at least 34 weeks till the baby's lungs are mature to sustain and breathe in the external environment. The ideal time for cerclage is between 16-18 weeks and it has to be done vaginally as its more saker than abdominally where it is attempted laparoscopically during this period.
The disadvantage of an abdominal cerclage remains that one may initiate uterine stimulation and uterine contractions and often during intra-abdominal operations, local injury to uterus may occur, which is not advisable. Vaginal route is still the safest and can be easily performed.

Appreciate for quick response. Are there any side effects for Cervical cerclage through vagina route? If we go this option, is it mandatory to go for this even in next pregnancy?

Hello,
there are no side effects of vaginal cerclage actually as it would have to be removed once she goes in labour and its easy to remove it vaginally. Also , if she has a normal delivery, then in next delivery as well, she would need to undergo the same procedure.
