Question: I want to have advice from a very good Sleep medicine doctor / Somnologist
My mother is 59 years old. All my mother's medical report are being enclosed. Pl go through the same first. She is having following problem
i) After doing some work she is getting tired and sleeping on bed is giving her comfort.
ii) She is not feeling any energetic to do any work and always liking to lie on bed very often
iii) During night and day time also, she is also not getting proper sleep.
During day time , even though she lies on bed and no sleep & remain awakened.
During night she sleeps at around 10pm & almost every night she woke up by 1-2 AM and thereafter no deep sleep and remain awake.
She has been prescribed triptomer but deep sleep occurs only once or twice in a week.
She is having a bit tension related to upcoming pregnancy related delivery of her daugher (my sister), i asked mother that do not take any tension for this & she confirm that i am not taking any tension for this.
I am worried as she is starved of deep sleep even after taking triptomer and what should be done.
Why this is so and I am worried.

Brief Answer:
Is she obese?

Detailed Answer:
Thanks for your question on 'Ask A Doctor' service.
I can understand your concern.
I have gone through the detailed history you have given.

Sorry to say but there is no attachment to your query.
So please attach reports again.

Also, let me know --

1. Her height
2. Her weight
In my opinion, she needs the Benzodiazepine group of the drug for sustainable sleep.
Alprazolam or Zolpidem will mostly help her.

I will be happy to help you further.
Wishing good health to your mother.
Thanks

Brief Answer:
Increasing age itself can cause sleep deprivation.

Detailed Answer:
Thanks for your follow up question on Ask a doctor forum.
I can understand your concern.
I have gone through the reports you have attached.

Answer to your 1st question.

Increasing age can itself cause sleep problems along with some hidden stress.

Answer to your 2nd question.

Yes, if not treated appropriately, this will definitely affect her life in long run.

Answer to your 3rd question.

The dose of sleep medicine can be reduced or even stop depending on improvement in patient's sleep in regular follow ups.

Answer to your 4th question.

Increasing age with underlying hidden stress might cause this.

Answer to your 5th question.

Yes. It can.

Answer to your 6th and 7th question.

I personally feel, she should definitely be evaluated by psychiatrist. Because they are masters in this kind of age and stress related sleep disorders.

Answer to your 8th question.

Somnologist or sleep specialist are majorly from Pulmonologist but few neurologists are also specialized in this field.

Hope I have solved your query. I will be happy to help you further. Wishing good health to your mother. Thanks.

Thank you for your kind advice.
I feel you have gone through all of my mother's medical report .
I) Reason for unexplained tiredness is not understood ? May pl see this.

Ii) regarding not having proper sleep (deprivation), age is understood but which is the reason (neurological /? ) and how will be treatment.

iii) How sleep study is done ? What is the cost ?

iv) You said to consult psychiatrist !
But my mother is not having sleep due to tension as per her statement. As she remain awaken , unnecessary petty petty thought comes to her mind . So sleep deprivation is first to blame for which there is no clear explanation .may pl elaborate
Will be thankful for your kind time.

I still believe it is stress and age both.

Detailed Answer:
Thanks for your question on Ask a doctor forum.
I can understand your concern.

Answer to your 1st question.

Unexplained Tiredness is also a feature of stress, depression and old age.

Answer to your 2nd question.

Treatment is by giving a pill which assist in sleep either sleep initiation or sleep continuation.

Answer to your 3rd question.

Cost of sleep study is around 10-12 thousand. It is done in sleep lab.

Answer to your 4th question.

Sleep specialist are basically deals with obstructive sleep apnea (OSA) due to obesity or any other obstructive upper Airway pathology. In your mother's case, I don't think sleep study will help. She needs proper counseling and consultation with psychiatrist and treatment ofcourse.

Hope I have solved your query. I will be happy to help you further. Wishing good health to your mother. Thanks.
