I M Into My 7th Day Of Taking Bactrim ,

Question: I'm into my 7th day of taking Bactrim, twice a day, for the symptoms described below. The first couple of days had me thinking I was going to have a quick recovery, but since then, it's been a roller coaster ride --- one day back to having the symptoms, the next day feeling better. Today I don't feel I've gotten much if any better. You can see the dialogue from last week with "Ask a Doctor". What should my next step be? Thank you.

For the past few days, I've had an almost constant prickling or tingling at the end of my penus...and it causes a constant feeling of the need to urinate.

I read your query and here is my advice.

The symptoms of discomfort in tip of penis and tingling sensation together with frequent need to urinate are symptoms of urinary track infection.
If possible do an urine analysis to confirm it or better am uroculture which also can show the type of antibiotic which is better used to fight it.
I would have treated you empirically with bactrim twice daily for 5 days.
Also you can use cranberry juice or tea found over the counter in pharmacy to help with disinfection.
Using ibuprofen is very useful to help with symptoms subside the discomfort and help with possible fever.

Thanks for query .

Based on the facts that you have posted tinglling at the tip of penis associated with urge to pass urine frequemtly is most likely to be due to UTI secondary to Benign Enlargement of Prostate which is common at this age .

Consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Serum PSA.
4) Blood sugar levels to rule out Diabetes .

In the mean time start taking broad spectrum antibiotics like Cefixime along with urinary antiseptic like Nitrofurantoin twice daily and urine alkaliser thrice daily.Later on switch on to appropriate antibiotics as per culture report.

Get the prescription of medicines from your family Physician.

Ensure to drink more water to keep urine dilute

.Further treatment will depend upon result of these tests and final diagnosis
