Question: I hooked up with someone last week, and since my testicles have been red and itchy, and there%E2%80%99s red splotches and bumps spreading to my leg. There%E2%80%99s white splotches on top of the red where the skin is flaking off. This is only the second person I%E2%80%99ve ever had sex with, and its been a long time since the first. Is it an STI?

I carefully read your query and also checked the uploaded image.

There are a few causes of a rash in the groin area. Generally, if a patient was exposed to unprotected intercourse the first thing to consider is genital herpes which in this case would indicate a sexually transmitted disease. It generally causes redness and blisters which latter burst and may create small round open areas of skin. These are generally painful as well (a burning sensation other than being itchy).

In this case, oral acyclovir is indicated (400 mg, every 8 hours for 5-7 days; different regiments exist, this is one of them).

Other possible causes but less likely in your case are a yeast infection or contact dermatitis. These just cause some redness and itchiness and the skin may flake as well, are not painful, and are generally not related to sexual activity.

I would recommend you to see your doctor and confirm the diagnosis. PCR test for herpes can be done using a sample from the rash. If the doctor thinks this is herpes then acyclovir (prescription drug) may be started and some tests are needed to rule out some other sexually transmitted disease such as chlamydia and gonorrhea and HIV, syphilis, Hepatitis B, etc
