Why Do I Have Pain In My Right Shoulder And Lower On The Right Side Of My Back?

Question: I have pain, based on position of upper body, in my right shoulder and lower on the right side of my back. It is quite likely that it is related to some heavy (too heavy) lifting I did s few days ago. I've had this before but it has gone away after a couple of days. This has hot and it are sleep very very difficult. Also, I am not able to take deep breaths as I experience the shoulder pain on deep breathing.

Hello,

The pain you describe is typical of pain due to muscular strain. Depending on the muscle being affected and the amount of the muscle fibers being hurt, the symptoms may vary in intensity and time till improvement. Typically it happens after a certain muscle effort, like it is happened in your case (too heavy lifting) or due to a certain quick movements and in this case you would feel the pain right at the time of the quick movement. Few times the patient does not even remember any event that caused the pain.

The muscular strain pain is worse on movement and if the muscles responsible for breathing are effected you will have difficulty breathing as well. Sleeping at night might be difficult till you find a position that is well with you but the moment you move it causes a sharp pain and you wake up and have little or no sleep.

To help with this pain I would recommend:
- paracetamol/ibuprofen both of them (after food), 4 times a day (take it regularly the first few days even if the pain feels good, unless you have problems with the stomach. Using both of them helps better with pain as they work with different mechanisms. If this is not enough adding codeine 15 mg every 6 hours (for 4-5 days only) would be of benefit, especially if you are having it hard to breathe.
- try to be active as much as you can (the daily activities) but avoid heavy exercise or weight lifting
- using local warm compresses might help relieve the muscle spasm and improve pain
- seeing a physiotherapist might be of help so he determines the muscles that are affected and in spasm and give you specific exercises to help them relax

The pain in muscular pain might take 1 day to go away and sometimes might take weeks. To prevent it from going long please be cautious so you do not hurt the muscles again and help them relax (medications, local warm compresses and physiotherapy)

You will need to see your doctor if no improvement with the above measures in 2-3 days or soon if any worsening.
