Question: I have mild gastritis but also determined to have both simple fatty liver through endoscopy and MRI.

I had also some blood tests to determine level of inflammation and fibrosis I believe; found to have no fibrosis and lowest possible level. However I have had kind of dull low level RUQ pain for 4 months! Which is the more likely cause of the pain??

Please give some additional information for me to assist you better:
What are the factors that increase the pain?
What are the factors that reduce the pain?
Relation with food?
Any referred pain to back, ribs, right shoulder?
Is stool normal? Does passing stool reduce pain?
Any pain or tenderness over right lower ribs?
Level of stress and/or anxiety? Do these increase pain?
Any urinary symptoms?

Any - HIDA scan for gall bladder function or MRCP (Magnetic resonance cholangiopancreatography) done till now?
What are the positive findings in previous investigations?
Any other additional information .

Detailed history and an actual physical examination also plays an important role in proper diagnosis hence please give detailed history question by question.

To date, mri of abdomen, and ultrasound of abdomen including gall bladder. Also, endoscopy and colonoscopy plus blood test checking for fibrosis etc in liver. Pain is always about a 3 out of 10 for 4 months. Seems to be a bit worse after eating no problem sleeping and milder pain when waking only a 2. No referred pain. Also some pain on lower right. Stool loose likely from long history of spastic colon. Maybe some reduction of pain after passing stool. Pain in RUQ just below ribs no pain above or tenderness. Level of stress anxiety and some depression much higher than normal. No other tests. Findings were polyp removed, mild gastritis, simple fatty liver. I have been using no alcohol for 4 months, Prilosec 40 mg for 3 weeks, FD Gard, fiber pills, cymbalta upped dose to 60 mg 2x, and hyoscyamine as needed. I’m no better and no worse. I’m guessing it’s mild gastritis or IBS but not improving!? Or could mild simple fatty liver cause these pains but if it were fatty liver would pain go away while sleeping or could it be my gallbladder not seen by ultrasound? I’m confused and a worrier. I should have added there was an incidental finding on a lumbar mri a couple of years ago of a right kidney cyst 10 cm x 8 cm x 8 cm. There were a few other kidney cysts but much smaller

My thoughts:
10 X 8 X 8 cm cyst of right kidney can cause dull pain in right side of abdomen as it is large.

Costochondritis can be considered a possible diagnosis if there is pain and tenderness in the lower ribs and the muscles attached to it, stress and anxiety have a direct relationship with this.

IBS may be a possibility since all the relevant investigations that you have mentioned are within normal limits. Spastic colon is usually due to IBS if not due to stricture or cancer - confirmed by colonoscopy.

Gastritis usually causes burning sensation in mid abdomen or on left side. Only duodenitis causes pain on just right side of mid line. And you area already taking medicines for this.

Gallbladder and bile ducts pains are enhanced by fatty meal once it reaches duodenum, is spasmodic, not known to occur continuously unless stone is impacted - MRI is normal.
MRCP is advised in such cases. HIDA scan also helps for gallbladder related issues.
Consult an Urologist to discuss about the large cyst in right kidney and its relation with pain, whether its excision will help for relief of pain.

Please discuss these issues with our Doctors to get additional investigations and additional medications or altered dosages.

Practically speaking, fatty liver does not cause pain in abdomen.
Pain comes on only on enlargement of liver that cause stretching its capsule which a fatty liver may not cause. Not seen any patient with pain in case of mild fatty liver till now.
