Question: I have had a scan showing a bulky uterus 102mm length x 51AP x 68mm width volume 187cc. There are 2 small subserosal fibroids and 2 paraovarian cysts. The problem is I am bleeding black type discharge and blood. I have been having bleeding inbetween periods for 3 months and at other times too even after a cervical smear. All tests came back normal. What do I do next as my bladder is under pressure and I'm at times nauseas and off color. Symptoms have been Dyspareunia intermenstrual bleeding and sometimes post-coital bleeding.

This is to add the description of the fibroids and ovarian cysts.

A 10mm anterior subserosal fibroid

Brief Answer:
Colposcopy and Hysteroscopy and biopsy are advised

Detailed Answer:
Hi,
Welcome to healthcare magic. I am Dr Ramadevi Wani. I will be answering your concerns today.

I have read your notes in detail.
Even though your Pap smear is normal, since you have persistent intermenstrual and postcoital bleeding, you need to consult your gynecologist and undergo Colposcopy.
You have not mentioned about the endometrium in ultrasound. Was it normal? It is better that you consult your doctor for hysteroscopy and biopsy.
Have you done the thyroid hormone test? Abnormal thyroid test also can cause abnormal bleeding.
Do reduce weight if you are overweight.
Further management depends upon the results of the above tests.

I will be happy to answer any further concerns that you may have.
Bedt wishes,
Dr Ramadevi Wani

The endometrium was 8.7mm

I examined myself last night and inside the vaginal walls black small clots right at the top of the back of the vaginal wall where I can feel bone there is also a lump that has grown. I forgot to mention as when I told my doctor she couldn't feel it so today when I see her I will squat as I can only feel it standing up or squatting.

The only other issue which started on the 4th December was the right leg pain like deep inside.

I will ask her to refer me for the further tests and diagnostic processes you recommended

My weight is 65kg and I am 5ft 5

Thank you very much

Brief Answer:
Please get examined by Gynecologist.

Detailed Answer:
Hi Miss XXXXXXX

Welcome back.
Endometrial thickness of 8.7 mm is normal in your case. Yet it is better to get a hysteroscopy with D&C and colposcopy done for the abnormal bleeding that you have.

The bulge that you have mentioned could just be a bulge due to bladder or rectum. Get examined by your doctor.
If you have further queries, do write to me.

Bedt wishes,
Dr Ramadevi Wani
