I Am Seeing My Doctor ASAP, As They Left A

Question: I am seeing my doctor ASAP, as they left a voicemail to make an appointment "as soon as possible would be great" (unfounterly I called back too late, they were closed for the weekend), following an ER visit on Thursday. I had shortness of breath, a sudden cough and severe ankle/foot swelling that's gotten progressively worse over the past 2 weeks.

I'm wondering if there's something in my blood tests that is of concern and since I have to wait until monday at the earliest, I'd like to know what the commulation of the following could mean. If need be, I can give the values. Thanks!

From the Venous Blood Gas:
HCO3-Line Draw is high
CO2 Content-Line Draw is high
Base Excess-Line Draw is high
Carboxuhgb-Line Draw iis high
Reduced Hemoglobin-Line Draw is 56.2; normal range is 0,0-5,0

From Differential
Neutrophils Absolute is high
Monocytes Absolute is high
Eosinophils Absolute is high

From CBC
WBC is high

Brief Answer:
Needs evaluation

Detailed Answer:
Hello XXXXXXX

After going through the medical details and the report details mentioned by you I would like to tell you that

1. Your symptoms are suggestive of cardiac or renal ailment.

2. The reports which you have detailed suggests infection

If you can upload the printed report then it will be better. Also few investigations will be required like ecg, Echocardiography and some blood tests for proper assessment.

Hope this information will guide you properly.

Kind Regards

Hello! I have uploaded screenshots of the reports in question. There was an ECG done, I was told it was normal. There were also x-rays done of my chest, I have no idea what the result of that is. The test done were:
BNP
Basic metabolic panel
CBC
D Dimer
Differential
Hepatic Function Panel
Pregnancy Qualitative, Serum
Troponin I
Urinalysis-Macroscopic w/Rfx to
Microsco
Venous blood gas, Line/Syringe

And the ones with the high results are the ones in the photos. The rest read as normal.

Does that give you any further information?

I Should mention, that my WBC, Neutrophils Absolute, Monocytes Absolute and
Eosinophils Absolute were high in July as well, so this appears to be an ongoing infection, or perhaps an autoimmune disease? Thoughts?

Sorry, I just wanted to say one more thing... is a Reduced Hemoglobin-Line Draw of 56.2 something unusual? Not being a medical professional, that result stood out to me as something to be concerned about.

Detailed Answer:
Hello again,
Thanks for the feedback...

Now taking into consideration the overall scenario which includes the normal morphology of all the blood cells normal relative blood cell count and slightly deranged absolute blood cell count I would rather like to tell you that your blood reports are within normal limits.

Furthermore, if you don’t have any history of ongoing or on and off fever, cough and cold or burning micturition chances of having any ongoing infection less likely.

The reason for the high values of reduced hemoglobin is because it’s a venous blood sample, not the arterial blood sample. Venous blood is basically deoxygenated blood and a major content of it is reduced hemoglobin that’s the reason why the values are on the higher side.

I hope this information will answer your queries. Feel free to ask back if you have any other doubts.

Kind Regards
Dr Bhanu Partap
