Question: I am an 86 year old male MRI results show moderate global parenchymal volume loss with an exvacuo dilation of the ventricles. Prominence of the sylvan fissures bilaterally with mild sulcal crowding near the vertex.
Faint smooth pachymeningeal thickening , which may be seen in intracranial hypotension or recent dural punction. I have symptoms of a CSF leak. What does this mean ?

The volume loss of brain is related to brain atrophy which is caused by the loss of the neurons.
Brain atrophy can be caused by disease such as Alzheimer or stroke and should be evaluated by a neurologist.
The condition of CSF leak is usually a medical emergency which needs immediate attention.
So, I suggest to see a neurologist as soon as possible.
