Brief Answer:
it could be

Detailed Answer:
Hello,

puffiness, weakness and the rest of the symptoms can be attributed to kidney disorders although this is not the only possible explanation for them. For example using a diuretic may cause electrolyte disorders, which may cause symptoms like yours. Low potassium causes weakness and low sodium causes nausea and other non-specific symptoms. Very low potassium can even cause flank pain by inducing rhabdomyolysis (destruction of muscle cells).

I cannot diagnose anything based on your symptoms only but these symptoms require investigation. I would have started with the basic tests: a urinalysis, a complete blood count, urea, creatinine, electrolytes, protein levels, creatinine phosphokinase, inflammation markers, etc. Regarding your flank pain an ultrasound scan of the urinary tract might help.

So in conclusion, your symptoms are serious enough to require investigation. Clinical examination and lab testing may reveal the cause.

I hope you find my comments helpful!
I'll be glad to provide more information or clarifications if you need any. In that case please be sure to send me more information about your medical history like the drugs you're taking, your diagnoses (besides diabetes that is), etc.

Kind regards!
