How Should Internal Hemorrhoids That Bleed Only During A Bowel Movement Be Treated And Cured?

Question: This question is for a Gastroenterologist please. Hello, I have internal hemorrhoids that only bleed when I have a B.M. Is there any way to stop this from happening? Thank you so much.

Hello,

* The internal hemorrhoids usually bleed through underlying veins from epithelial damage via hard stools.

* As per my long surgical experience, I recommend you to do the following for best control of bleeding during bowel movement.

- Maintain hydration with plenty of liquids, essential for smooth bowel movements
- Consume high fiber diet to avoid hard stool
- Avoid all spices, non veg, dairy fat, junk foods
- Avoid smoking or alcohol if consuming
- Regular basis sitz bath (sitting in a tub filled with lukewarm water added epsom salt)
- Local application of anti hemorrhoid cream in case of itching and swelling
- Wise intake of laxative tablet, syrup or bulk forming agent as per guidance of your family doctor
- Avoid secondary parameters as over the counter use of pain killer or else which can precipitate blood in stool frequently

* If all conventional measures which are keeping stool soft and still there is bleed, you must discuss the condition with your consultant surgeon for possible need of laser or other procedure to cure it.

Hello and thank you for responding. Are you a Gastroenterologist? Per the Bristol Scale, my stool is formed like a ripe banana, which I understand is how it is supposed to be. They are not hard at all. I have had a problem with constipation in the past, which is how I suppose I got the hemorrhoids to begin with. These internal hemorrhoids bleed whenever I have a normal B.M. – I am not even straining. There is no pain or swelling. However, this bleeding is causing my hemoglobin levels to fall. There has to be a way to stop this from happening. I already do/have done everything on your list. Hopefully, you have some other ideas. Thank you.

* If you have regular bleed with normal consistency stool without straining, my surgical experience suggests

- Prescription of specific molecules that control capillary bleeding as calcium dobesilate or others from your consultant (which will give primary relief for some time)
- Definite procedure in terms of laser, staple or other procedure as per your Consultant Surgeon`s choice

Hello and again, thank you so much for replying. What would be the name of the medication that you have suggested please? Also, In your opinion, do you think that Staples would be beneficial? On a different note, can the drug Carafate be taken on a long term basis? Thank you so much and I look forward to hearing back from you soon.

Hi,

* The trade names vary depending upon the location, Calcium Dobesilate is generic name; I can suggest Vasculflow or Doxium as trade names, but again a prescription drug requires help from your consultant only.

* Staples or laser are not only beneficial, they are curative when the bleed is definitely from the sole source of hemorrhoid.

* The Carafate can be taken for prescribed period of time for its therapeutic purpose. Beyond certain time period the usage of which may be a threat for body to make your body cells not responsive to the drug at all or other side effects, so I do not recommend the same as per my clinical experience.
