Question: Hi Sir/Madam,
I stay in XXXXXXX Over the past 3 years, during winter (Nov-Feb), my ears get blocked. My hearing gets reduced. Problem is more in mornings. I also get frequent sneezing in night. I have to cover myself from head to toe to reduce cold air affecting me. Please guide. (Along with me, other family members are also getting affected but in some other way like my father getting heavy chest congestion, children coughing in night, etc)

Hi,

Banglore is one of the highly polluted cities.
I came across so many patients who developed these kind of symptoms especially after staying in XXXXXXX
So these king of allergic symptoms, sneezing, blocked ears, chest congestion, coughing etc are mostly due to allergy.
You might be having allergic rhinitis which is causing frequent sneezing, running nose, blocked nose and ears etc.
So better to start
1. Intranasal spray containing azelastin and fluticasone. Two puffs twice daily in each nostril.
2. Oral combination of antihistamine (levocetrizine or fexofenadine) and anti allergic (montelukast) once a day at night.
3. Use air purifier at home and office. This is very useful.

Thank you sir for providing me a specific solution. It greatly helps!
I just wanted to add couple of facts which i forgot to put in question:
1. If i eat curd / juice / cold water in rainly or winter season, i get some wheezing around 3am-4.30am the next day (it keeps me awake for 1-1.5 hr). I stop eating cold or juice items, but some times just having little bit of curd triggers it. It started roughly 3-4 years back. I have been in XXXXXXX from 14 years.
2. I get sneezing from cold air / dust as well.
3. Kindly suggest what is the duration of these medicines. Should i take them for few days in winter and stop them when problem stops?

You are also having allergic asthma.

Detailed Answer:

Hi,

These symptoms of yours are more suggestive of allergic asthma.
So better to consult pulmonologist and get done clinical examination of respiratory system and PFT (Pulmonary Function Test).
If PFT is showing obstructive defect then you will need inhaled bronchodilators (formoterol or salmeterol) and inhaled corticosteroid (ICS) (budesonide or fluticasone).
There is no particular duration of treatment.
You should first start treatment and see for improvement.
If you are improving then we can gradually taper down drugs.
Once all your symptoms are reduced, you can take medicine as and when required basis.
Please let me know if you have done PFT in the past.
