Question: Hi, my problem has been steadily getting worse. I am a 65 year old female with metformin controlled diabetes as my major disease. For a month or two I have been developing a pain in my lower left abdomen, some bloating, urethral pain when peeing, constipation and/or pain when straining, sometimes diarrhea but other times very regular. I take metformn, citalopram, lorazapem, trazadone, valacyclovir and pravastatin.

Hi,

Thanks for your query.

Diabetes is a immunocompromised state, the chances of infections are relatively more.

Since you have urethral pain while urination, this is an indication of urinary tract infection which has to be ruled out by getting a Urine microscopy test.

The pain in the lower abdomen could either be referred pain due to urinary tract infection or could be bladder infection altogether.

I also recommend a Ultrasound scan of the abdomen which will rule out any other pathological conditions which can present with above symptoms.

About the loose stools , I would like to know duration of your diabetes and whether do you face constipation alternated with loose stools.

I was diagnosed with diabetes about three years ago. I had to switch to slow release metformin because I was constantly getting diarrhea with daily use. The slow release is ok but I sometimes stop it for a day or two if I get diarrhea again. Sometimes I need nothing because I'm super regular without stool softeners. And sometimes these are needed. So, rotation not so much but it does alternate somewhat.

I had a colonscopy in July that showed diverticulitis and about four benign polyps. I had an abdominal scan earlier this summer that showed a fatty liver.
