Question: Hi,
I have had microhematuria and following up with imaging. Since I've had 10+ ct's of my lungs for 17 nodules over the past 8 yrs I'm reluctant to do ct because of radiation. I'm also concerned about contrast nephropathy as my efgr has been between 60-90 after 35 yrs of hbp treatment. My insurance company has rejected mri urography and I wonder what reason(s) to present in my appeal. I do have access to tesla 3 mri and understand sensitivity is similar and that mri might be advantageous for small renal masses(Uptodate-Small Renal Masses". My son died from clear cell kidney cancer and I'm an exsmoker who was exposed to benzene and hydrocarbons when I was in my twenties. Thanks for all you do.

Hello,

the only valid reasons would be either a prior allergic reaction to CT constrast media or superiority of MRI over CT - adequately documented by your doctor. Since the first step in microscopic hematuria investigation is usually the CT scan I doubt that any insurance company would accept the MRI (at least not before the CT scan).
