Question: Hi. I have been on here before with bouts of anxiety in the past and have been having new issues that I am concerned about. A few months ago, I started feeling like I would get hot and dizzy sometimes. In early January I started getting a pain in my left abdomen that I worried was my appendix. It was ended up resolving but since then I everything has been spiraling. I had some headaches that I was concerned about and all the worrying made my period 2 weeks late. After getting a false positive pregnancy test (confirmed by my gyno) I was extremely stressed for a few weeks. Anyway, I have been getting bouts of dizziness every now and then and a few days ago, had some tightness in my upper back. Two nights ago, I visited an ER because I woke up with a pain in my upper left chest. It was a light pain that would come for a couple of seconds and then go away for a few seconds. My ECG ended up being normal and monitored for a while. The blood work came out normal with my Troponin (<0.05), myoglobin (36.4) and CKMB ((<1.0)levels being normal. They sent me home but now I am concerned that they did the blood work when I got there and it wasn’t long enough to see the numbers increase. I’m not sure what to do. I did see my PCP early last week and all of my regular blood work was normal. I’m worried. Could they have missed something?

Brief Answer:
The test results are normal.

Detailed Answer:
Hello dear, welcome to Ask a doctor's service.
I read your query and here is my advice.

I see the test results you uploaded and they are all within normal levels.
I think you don't have to worry about it.
Regarding to the previous episode of chest pain I think they did the tests at the right time because CKMB, troponin and myoglobin are done early in time . Also your ECG was normal and when you went home you had no more symptoms.
I believe it can be muscle pain or even stomach pain.
Also anxiety may contribute to these symptoms you are recently heaving.

Hope I have answered the question.

Thank you. I’ve read a few things that indicate that troponin doesn’t increase until 3 or so hours after a heart attack. Is it possible that it was still low because I went to the Er right when I felt a pain?

Brief Answer:
I don't think you are missing anything.

Detailed Answer:
Hello dear, thank you for asking again. It is true that troponin and ckmb start to increase 3 hours after the starting of chest pain while myoglobin 2 hours. Still I don't think they missed something because they monitored you which means wait and see. If you don't develop other symptoms or if chest pain doesn't become worse it isn't related to a heart attack.
If you continued to have chest pain they may have tested again ckmb , troponin and myoglobin to compare the initial results.
Also you went home and you had no symptoms so I don't think that this situation is related to heart issues.
Let me know if I can assist you further.
