Question: Hi
I have a high-pitched sound coming out of my ear when I open my mouth to the extreme and I particularly notice it while chewing, yawning and brushing my teeth. I also have some fullness when my jaw is still and when I move my head sideways in one direction. These symptoms have now been troubling me for about 11 months. I have previously seen an ENT doctor who thought there was mucus lodged in the eustachian tube but surely it should have cleared by now. What could be causing it and how can it be treated please?

Hii. Welcome to the health care services. It seems that you may be having eustachian tube dysfunction, TM joint arthritis or sensorineural hearing loss. You should do an audiogram and impedance audiogram. Get the hearing status checked. Weakness in the cochlear nerve can manifest with sensorineural hearing loss and aural fullness. With the help of audiogram we can diagnose hearing loss and eustachian tube dysfunction. TM joint arthritis can also manifest with similar problem. Do an orthopantomogram and consult a dentist. Apply hot fomentation over TM joint. Do chewing movements or chew bubble gum. Start jaw exercise. Start a fluticasone nasal spray and steam inhalation and continue for at least 1 month. Ignore the sound. Most of these sound can be idiopathic. Do not stay in silent area. Review with audiogram and if you have any queries.

I have seen an ENT doctor who has already performed a heating test which came back as almost normal.

My doctor thought that I was suffering from temporomandibular joint dysfunction because I particularly notice a high-pitched sound when I move my jaw up and down such as while chewing food and yawning. I have now had these symptoms for almost a year. Will these symptoms eventually disappear? Many thanks.

Hii. These symptoms should disappear over time but it may take a long time. You may need to start jaw exercises. There are many therabite devices available nowadays for treatment of temporomandibular joint dysfunction. Give hot fomentation over TM joint.Have patience.

Hii. If you are having TM joint dysfunction or eustachian tube dysfunction then some sensation of aural pressure is normal. But if you are having giddiness also then kindly do an ENG or electronystagmogram to see if there is any vestibular dysfunction. Do a MRI brain if you are too much worried about the sound to see if there is any abnormality in the brain which can rarely produce that sound.
