Hi , I Am Suffering From Burning In Whole Abdomen

Question: Hi ,

I am suffering from burning in whole abdomen for past 5 months now. No other symptom. No unusual bowel movement , only stomach burning sensation. I eat normally and there is no change in my appetite. I am 33 , do not smoke , drink once in a week. My BMI is around 30. I work in an IT job and i have some side business due to which i am busy for almost all days of the week.

I saw a gastroenterologist in XXXXXXX in September who told me to get some tests done. So i had blood tests related to Amylase/lypase , liver function tests, stool related tests, c reactive protein and USG whole abdomen. All of them came out to be normal. There was some elevation in SGOT and SGPT tests but i always get them as i have fatty liver. After that the doctor prescribed me nexpro-40 mg twice a day. I have been taking them regulary. I was also told to reduce spicy stuff from my diet and i have almost stopped eating food with spices. I do not use chilly at all.

Inspite of all this right now what i have observed that the stomach burning is only lesser or negligible when i do not consume spicy food and take nexpro-40 regularly. As soon as i stop taking the medicine or eat even a little spicy food my stomach burning restarts. SInce i am too much involved in my buisness right now i am not able to go to the doctor every now and then. But i have not even been told what my condition is. My doctor only advised me to change my dietary habits. Even after changing them i am not able to see a permanent change.

Please let me know what this condition is and what more can i do to get this resolved.

Brief Answer:
Gastroesophageal reflux disease with gastritis.

Detailed Answer:
Hello and thanks for choosing "Ask a Doctor" for your query,
Have seen your details and very likely you are suffering from GERD and acid peptic disorder.
Gastroesophageal reflux disease, or GERD, is a digestive disorder that affects the lower esophageal sphincter (LES), the ring of muscle between the esophagus and stomach. Reflux means to flow back or return. Therefore, gastroesophageal reflux is the return of the stomach contents back up into the esophagus. The backwash (reflux) irritates the lining of your esophagus and causes GERD.In normal digestion, the lower esophageal sphincter (LES) opens to allow food to pass into the stomach and closes to prevent food and acidic stomach juices from flowing back into the esophagus. Gastroesophageal reflux occurs when the XXXXXXX is weak or relaxes inappropriately, allowing the stomach's contents to flow up into the esophagus. If not treated, chronic acid reflux can injure the esophagus and, in some people, progress to a precancerous condition known as Barrett’s esophagus.

Usually we manage GERD with a combination of medication and lifestyle changes. If symptoms persist or are severe, innovative endoscopic and surgical procedures can be done that provide long-term symptom relief.

Use antacids like omeperazole or pantoprazole before . In my clinic, I suggest 40 mg twice daily.
Using motility agents like domperidone will help with rapid stomach emptying and decreased refurge or heart buring.

Don't lye flat right after meals but take brisk walk or stay upright for almost half an hour after meals.

Avoid spicy, greasy, oily foods as well as fizzy drinks. Avoid too much quantity if hot beverages.
If not relieved in two weeks go for endoscopy test.

Hello Sir ,

Thanks a lot for your detailed explanation.

When i am eating right and taking medicine i feel good. As soon as i eat even a little outside the burning comes back and if i stop taking medicine it becomes worse.

My other worry is i have been using nexpro-40 daily for the last 5 months. I take morning dose regularly and sometimes skip the evening dose. Should i be taking the meds for such long? Since this is 5 months already can i go for an endoscopy right away ?

Brief Answer:
You can go for endoscopy.

Detailed Answer:
Hi again,
When you eat food acid is secreted for digestion. This causes pain,Antacid neutralises the acid hence you feel better.
Nexpro optimal dose is 40 mg twice daily and can be taken for without any significant side effect(please don't go on Dr Google information).
Thank you
