Question: Hi, I am suffering from abdominal pain and discomfort, feeling stomach bloated, moving burning pain in stomach, also feeling burning pain and pressure under ribs. Feeling sensitive to touch that area. Sometimes feeling stiffness and heaviness in chest with mild burning sensation.
I have done endoscopy & biopsy recently and report were normal. H Pyroli test is also negative. Only small hiatal hernia found in report where no treatment is required for this as suggested as gastroenterology.

Hello Sir,
PFA biopsy report. As I checked with UK doctor, he did find any reason to rule out eosinophilic esophagitis. He checked gerdia infection during biopsy.
Finally he concluded your symptoms could be cause of IBS, visceral hypersensitivity, Nerve pain or functional dyspepsia. He prescribed amitriptyline but I have not started to take it.

Previously I had suffered from acid reflux and IBS( Morning diarrahea) also.
One year ago I had suffered reflux rebound due to long term PPI treatment when I suddenly stopped to take it that caused much burning pain in stomach or chest but currently not having that much burning pain but sometimes suddenly feeling tenderness,pressure and burning on rib cage, chest pain, feeling stuffiness and pain in center of chest and breastbone burning sensation but it is not continuously.

Feeling pressure and burning pain on rib cages either on left or right mostly during sitting work. Sometimes rib cages pain having while walking also and during exercise also feel it but it is not happening continuously.

Currently I am taking Cod liver oil( which contains vitamin A and D) and sometimes I am taking probiotic and gaviscon advance. As well as using anti histamine and nasal spray if in case of continuous sneezing and runny nose due to allergy rhinitis.

I am not sure whether whether I have some vitamin deficiency like vitamin D and B12.
I have tried to search the cause of my symptoms in google. I have found few of them as given below but not sure that is linked to my problem.
Costochondritis
Fibromyalgia
Intercostal Neuralgia ( Nerve Pain)
Autoimmune disease.
Vitamin defeciency ( D or B12)

Hello Sir,

One more thing I would like to highlight you whenever I going for poo my stool is loose, not watery whereas the frequency of motion is once a day and sometimes twice a day. But if I will consume too much water in morning then it is causing watery motion. Doctor examined IBD test also few months back but that was also negative.

Hi,
Yes, as your doctor said, you may be having functional bowel disease.
You need to start taking amitriptyline.
You also need to take probiotic VSL#3 and low FODMAP diet.
If test for celiac disease not done before, kindly get it done.
As far as the chest symptoms are concerned, you will get relief with PPI and amitriptyline. So start taking PPI again. If there is no relief. then esophageal biopsy may be done to rule out eosinophilic esophagitis. Since you have history of allergy, eosinophilic esophagitis is still a possibility as it is also allergy related.

Hi Sir,

I can start PPI and amitriptyline but since I have a bitter experience with PPI when I stopped it like reflux rebound effect so I have a question like how long should I take PPI and how to wean off it. I am also worrying about amitriptyline side effects and how long should I take it and how to wean off it. Please suggest.

Also please me know the alternative test to rule out eosinophilic esophagitis.

Hi,
Your can take both for 4 weeks and we can plan weaning them off after that.
Biopsy is the only way to confirm the diagnosis of eosinophilic esophagitis. But it is necessary only if you are not responding to the above mentioned medications.
