Question: Hi Doctors,
Patient: Female 64 years
Had an allergic reaction when traveling and had skin rashes all over with itchiness. They subsided with usual medicine but then she developed cough and AEC was 1750 and then was prescribed
Hetrazam thrice a day 14 days
Pregalia once for 30 days
Zental 400 mg one

The cough didn't reduce so had ayurvedic syrup. But continued with medication with out fail.
The cough has almost gone since the last visit on August 29th. 2020. But today the AEC has shot up to 2640 compared to 1750 on the day of the visit.
Although she seems much healthier than before.
What should she take for lowering the AEC?

I can understand your concern.
Raised AEC count is suggestive of allergic etiology.
According to her symptoms, we should definitely rule out allergic bronchitis.
So get done serum IgE, chest x ray and PFT (Pulmonary Function Test).
She should be started with montelukast and fexofenadine combination.

Serum IGE 1175 IU/ml
AEC is 1380 cells/cumm
There is minimal congestion of lungs unlike before.
And you prescribed but did not mention when and how many to take. (Montelukast combined with fexofenadine.
Chest xray
Cvp line is noted
Inhomogeneous opacity noted in the right para cardiac region
Elevated right hemi diaphragm appears normal,
Visualized bones and soft tissue appear normal.
Could not do a PFT due to covid and accessibility reasons. But slight shortness of breath while climbing the second flight of stairs.

You should take montelukast and fexofenadine combination once a day at night before going to sleep.
Take for 30 days.
