Hey Doctor, I’ve Been Having Dysuria For About A Year

Question: Hey doctor,

I’ve been having dysuria for about a year on and off now which consists of a burning sensation when I urinate. I can feel it as the urine first comes out of the bladder not actually in my urethra which is what I originally thought was happening. The pain is also especially worse if I don’t drink enough water and I’m not really a big water drinker. Now I have had a bunch of blood tests and urine samples sent off which all showed no signs of infection. I also visited a urologist about 3 months ago about it and he kind of palmed it off being anything especially since I’ve repeated the urine tests several times and have no “alarming” symptoms. Also when I first got this I had never had sex before so I’ve had this for about 10 months prior to ever having sex with anyone the only thing I ever used was a fleshlight sex toy but I’m unsure if that can cause anything? Is there anyway the current tests I’ve done have missed anything, is there any further testing that I should be doing can I possibly have an infection without it showing up in tests?

In terms of medication I tried ural a couple of times but it didn’t do anything at all could it maybe be something other than an infection can you have symptoms of dysuria and it not actually be related to anything serious in nature?

Thanks for query .

Based on the facts that you have posted you have dysuria since one year and all the relevant urine tests have not revealed any evidence of infection .

I shall be happy if you can post me more details to help me in making precise comments
1) Do you have pain in perineal region ?
2) Do you have pain while ejaculating ?
3) Have you done urine culture at any given time ?

The long standing Dysuria could be due to non bacterial Prostatitis ,

Please get following tests done to help to establish diagnosis
1) Semen Examination and Semen culture
2) Microscopic examination of a urethral discharge obtained after doing Prostate Massage
3) Ultrasound Scanning of abdomen and pelvis.

In the mean time start taking Doxicycline twice daily for two weeks . .

Ensure to drink more water to keep your urine dilute .This will help to reduce dysuria.

Sometimes I feel a bit of an ache in my lower pelvic region when masterbating but not during sex but nothing too painful.

Yeah I’ve done multiple urine cultures which were all negative for anything.

I haven’t done any of those tests you’ve listed but I think mentioned them to my gp but she couldn’t do them, does this have to be done through a specialist? I will get the ultrasound done though.

In terms of non bacterial Prostatitis what’s the main treatment for this? Is it bad that I have had it un diagnosed and un treated this long? Can it increase chances of getting cancer etc? With the antibiotics my doctor has avoided giving them as I have a really bad stomach and gut and they upset them badly

Thanks for that doctor I will do however in terms of it being long standing is that a bad thing in my case considering my age? I’m just away from home and won’t be back for another week would the wait make much difference in the last year my symptoms haven’t really changed they’ve stayed the same.

Hey doctor last night I noticed a slight itch on the underside of my penis and upon checking there was a red like rash that had developed. I’ve never had anything like this before and I noticed it after having a shower. Now a couple of things I am overseas at the moment and I have been using a different soap the last couple of days so I am unsure if this caused it? But there is also some scaly bits on the scrotum and penis skin as well. Based on the images what do you think is the cause of this? I have really bad health anxiety so I am a little freaked out especially being overseas. In terms of STD’s I am unsure if it is that? I have only ever had two partners I’ve slept with and one of them is currently my girlfriend and I know she doesn’t have anything and the first one was over 3 months ago and didn’t have anything and I have always wore condoms. I’m unsure if I will be able to visit a doctor until I get back on the Monday next week is this a bad thing? I can possibly visit one here but might be a bit hard.

Upon waking up the next day the redness has gone completely and now there is just a dry white patch and flaky brown patch. What do you think this can be?

How exactly does one get jock itch is it common for men to get and how long would it take to heal up do you think?
