Question: Hey doctor I was recently prescribed Fluconazole 150mg just the one off dose however after reading through the leaflet it warns about taking it with heart conditions. I have this asymptomatic heart condition called pre-excitation so I was wondering if it were still okay to take the one off tablet for the fungal infection my doctor seemed unsure.

Detailed Answer:
Hello

After going through the medical details provided are you I can understand your concern and I would like to tell you that fluconazole should be avoided if the patient is already taking drugs that prolong the QT interval like cisapride, quinidine, amiodarone etc.

Are you taking any anti arrhythmic meds? If yes kindly specify.

Per se taking fluconazole once in a while in pre excitation doesn’t cause any harm.

Kind Regards

Hey doctor thanks for the reply I’m currently not taking any anti arrhythmic and have never taken them either however do any natural supplements effect it the heart rythum in any way or is just the pharmaceutical ones that are the cause for concern? I’m currently taking some natural supplement powders for my gut which I have attached images of but other than that I’m not taking anything else would this be okay?

I have reviewed all the images attached and these are all herbal supplements which don’t have any direct effect and no proven scientific studies on pre excitation.

How you know you have pre excitation?

No it’s not advisable to consume alcohol while taking medications of any sort. Also alcohol can affect heart in long term.

Fluconazole can be avoided. Yes there are many drugs to be avoided but that’s not important as of now because you are not on any medications and just remember to tell your treating doctor before hand whoever you are getting any prescriptions that’s all.

Hey doctor thanks for the reply I had an ECG done and it was found incidentally I then underwent further testing and it was confirmed however I don’t have any symptoms so the cardiologist I saw said to leave it be. So it would be safe to take the one Fluconazole 150mg and I shouldn’t have too many dramas? Can you still consume alcohol in any capacity while on the medication as well?

Please upload the image of your ecg so that I can be sure that it’s pre excitation only and not prolonged qt interval.

I want to see and confirm because fluconazole is better avoided in prolonged qt.

Hey doctor I don’t have a copy of my ECG but I just spoke to my cardiologist and he said it was all good to take it and also in the letter about my diagnosis it specifically says I have “asymptomatic pre-excitation” so that’s what’s I’ve got in terms of my heart so that’s prolonged qt interval is it?

If you have irritation and redness then I would suggest you to take some anti allergic like levocetrizine 5mg one tab at night for 3 days and apply lactocalamine lotion twice daily.

Hey doctor just a follow up with this I took the Fluconazole 150mg on Wednesday last week however I had sex last night with a condom and the area has gone red again just the glans mainly it looks like Balanitis. Is there anyway to calm the irritation and redness considering I am already on the Fluconazole would it still be working at this stage or would I need more? Would using warm water with salt in a plastic cup for a bit help it I’ve read online salt baths would help but I assume they would do the same thing? It’s just really irritating is there anything I can do?
